WATERBURY, Vt. — The Vermont State Police are investigating a fatal two-vehicle crash that occurred Friday afternoon on VT Route 100 in Waterbury.
At approximately 1:32 p.m. on February 28, 2025, troopers from the Berlin Barracks responded to reports of a crash in front of Billings Mobile. Upon arrival, they found a head-on collision involving a 2011 GMC Sierra and a 2017 Western Star truck.
According to preliminary findings, the Western Star, driven by 41-year-old David J. Brier of Saint Albans, was traveling south on VT Route 100. As Brier proceeded through the intersection on a green light, the GMC Sierra, driven by 56-year-old Penny Mandigo of Waterbury, veered across the center of the road and collided head-on with the truck in the southbound lane.
Mandigo, who was not wearing a seatbelt, suffered fatal injuries and was pronounced deceased at the scene. Brier, who was wearing a seatbelt, was uninjured.
Emergency responders from the Waterbury Fire Department, Waterbury Ambulance Service, and the Vermont Department of Motor Vehicles assisted at the scene.
The cause of the crash remains under investigation, and authorities are asking anyone who witnessed the incident or has relevant information to contact Trooper Mae Murdock at the Vermont State Police Berlin Barracks at 802-229-9191.
